Mr. Charles W. "Chuck" Moore, Jr., age 62, of Mayfield passed away on Tuesday, June 2, 2026 at Mills Health and Rehab Center in Mayfield, KY.

He was a member of Mayfield First United Methodist Church, a 1984 graduate of Mayfield High School, an avid bowler and a NASCAR fan.

Mr. Moore is survived by his mother, Betty Stallins (Pedro) Reyes of Mayfield, KY; his father, Charles W. (Linda) Moore, Sr. of St. John, IN; one sister, Monica Moore (Scott) Burton of Phoenix, AZ; three nieces, Chelsey Burton, Elizabeth Burton and Rachel Andrews; one nephew, Grayson Andrews; one great niece, Violet; one great nephew, Ronnie; and by several aunts, uncles, cousins and friends.

He was preceded in death by one sister, Rhonda Andrews and his stepfather, Charles Stallins.

A memorial service for Mr. Charles W. "Chuck" Moore, Jr. will be 6:00 p.m. on Thursday, June 4, 2026 at Mayfield First United Methodist Church, 1322 W. Broadway, Mayfield, KY 42066. Rev. Adam Rush and Rev. Mark Stephens will officiate.

Visitation will be 5:00 - 6:00 p.m. on Thursday at the church.

There will be a reception after the service.
